Each year, health screenings are completed on every student which includes height, weight, and vision.  Students in 7th grade will also have their hearing screened and  their spines checked for scoliosis.  Their health screening results will be posted to Sapphire once completed.

MEDICATION ADMINISTRATION:
Medication Permission Form

It is the policy of Central Columbia School District to administer prescribed medication during school hours when necessary.  If your student requires medication during school on a daily basis or as needed (epi pens, ADD/ADHD meds, epilepsy meds, inhalers, etc.) please return this completed form to the school nurse.  The prescribing doctor can also fax a medication order to me at the number above.  Medication will not be given without the doctor's order and permission form signed by a parent/guardian.

6th Graders
Physical Form

All students in 6th grade are required to have an updated physical for school.  This physical can be completed by your family doctor or our school doctor.  If you would like your family doctor to complete it, please have him/her fill out the physical form and return it to the school nurse. If your child receives any immunizations during this physical, please provide an updated vaccination record.

If you would like your child to be seen by our school doctor, please send me an email and I will add him/her to the list.  Please note, we do not give immunizations during school physicals and all 7th grade students are required to have a TDAP and MCV vaccine prior to the first day of school.  Your student will need to be seen by an outside provider to receive these vaccinations for next year.  Please provide documentation afterwards.

7th graders:
Dental Form

All 7th grade students are required to have a TDAP and MCV vaccine prior to the first day of school.  Most of the time they are given during the 6th grade physical.  Please provide an updated immunization record once the vaccines are given. 

All students in 7th grade are required to have a dental exam.  This exam can be completed by your family dentist or our mobile dentist, Smile PA, when they visit our school  If you'd like your student to be seen by Smile PA, please visit www.myschooldentist.com to enroll them in this service.
